Cautions
Before and during therapy are recommended regular blood tests (cellular elements) and urine monitoring indicators of liver function. With caution is prescribed for the presence of a history of heart disease, liver or kidney disease, with hematological disorders, increased intraocular pressure, latent psychosis, inadequate response to external stimuli, anxiety, illness, characterized by spasms of mixed character, the elderly, drivers of vehicles and persons exploiting mechanisms. You should not abruptly discontinue therapy. Women are encouraged to additional folic acid (before the onset of pregnancy or during her) in order to prevent bleeding disorders in the last weeks of pregnancy and in newborns may use vitamin K.
Overdose
Symptoms: disorientation, drowsiness, agitation, hallucinations and coma, blurred vision, dysarthria, nystagmus, ataxia, dyskinesia, giper-/giporefleksiya, seizures, myoclonus, hypothermia, respiratory depression, pulmonary edema, tachycardia, gipo-/gipertenziya, heart failure, accompanied by loss of consciousness, vomiting, decrease in motility of the colon, fluid retention, oligouriya or anuria, changes in laboratory parameters: hyponatremia, possible metabolic acidosis, hyperglycemia, increase muscle fraction kreatininfosfokinazy.
Treatment: induction of vomiting or gastric lavage, the appointment of activated charcoal and saline laxatives, forced diuresis.
To maintain the airway - intubation of the trachea, artificial respiration and (or) the use of oxygen.
If hypotension or shock - Plasma dopamine or dobutamine, with the appearance of convulsions - the introduction of benzodiazepines (diazepam) or other anticonvulsants (in children may be increased respiratory depression in the development of hyponatremia - water deprivation, cautious in / infusion of isotonic sodium chloride solution.
With a combination of severe poisoning with renal failure shows renal dialysis. The specific antidote is available.
It should anticipate the re-intensification of symptoms of overdose for 2 nd and 3 rd day after it began, owing to the slow absorption of the drug.
Drug Interactions
Incompatible with MAO inhibitors. Increases hepatotoxicity of isoniazid. Decreases effects of anticoagulants, anticonvulsants (hydantoin or succinimide derivatives), barbiturates, clonazepam, primidona, valproic acid. Phenothiazines, pimozid, thioxanthenes increase CNS depression (decreased convulsive threshold), cimetidine, clarithromycin, diltiazem, verapamil, erythromycin, propoxyphene reduce metabolism (increases the risk of toxic action). It lowers the activity of corticosteroids, estrogens and estrogensoderzhaschih oral contraceptives, quinidine, cardiac glycosides (induction of metabolism). Against the background of carbonic anhydrase inhibitors increases the risk of violations of osteogenesis.
